AGENT REFERRAL FORM

Submission of this form constitutes an agreement between NBI SUNCONTROL and the form’s submitter.

NBI SUNCONTROL agrees to pay the undersigned agent a 10% commission based on the gross sale for any referral that results in the sale of 3M Window Films. This agreement applies only to sales equal or greater than $300 with the maximum commission not to exceed $1,000. Commission check will be made payable to the Company listed above unless otherwise noted and will be paid within 30 days of installation.

This agreement implies no obligation to NBI SUNCONTROL on the part of the undersigned agent.
